The directory is for health professionals and people with diabetes to identify resources in their community; resources are listed by county and have information about the program, audience served, languages available, and how to contact the program. Three National organizations accredit or recognize diabetes education programs to ensure that they are following National standards on diabetes self management training. In addition to recognized/accredited programs, many community based organizations offer diabetes education classes.

Chronic Disease Self Management Education – Arizona Living Well
People who have diabetes need skills to manage their diabetes and to help them stay active. The Arizona Living Well Program is a highly interactive, six-week workshop led by professional trainers in community settings for people with chronic health problems.
The program builds confidence for participants through mutual support and helps people succeed in leading active and fulfilling lives. The program is also offered in Spanish: Tomando Control de su Salud.
